The most important role of the Breast Health Patient Navigation Program is to assure that every person receives a timely diagnosis and treatment. Our goal is to assist with the coordination of care and services and help guide you through the health care system.

 

The Breast Health Patient Navigation Program can:

 

  1. Direct you to health care services for further diagnosis and treatment
  2. Provide help with insurance questions
  3. Assist with coordination of care and help guide you through the health care system
  4. Offer information and referrals regarding insurance options if you don't have insurance
  5. Provide information about local transportation resources
  6. Supply referrals and information regarding local resources, supports and services.
  7. Help you get answers to your questions
